The Occupational Therapy Assessment and Report

The journey begins with a comprehensive evaluation conducted by one of our experienced occupational therapists. During this initial assessment, we take the time to understand your concerns, challenges, and goals. Knowing this important information will enable us to create a personalized report targeting your requirements.

Our Occupational therapist work with the client to create a safe and hazard free home environment that promotes and maintains independent living for as long as possible.

An Occupational Therapy Assessment and Report is necessary if you wish to utilize Funding through the NDIS or Home Care Package scheme.

Whats the difference between a home and an equipment assessment? We break it down below.

Following an assessment Summit Health arranges an Equipment Trial with the client to ensure that the correct equipment is chosen before purchased.

Home Assessment

This assessment is completed at the clients home. We look at the entire house during the assessment however are guided by the individuals needs to maintain mobility and independence.

  • Access and exit to home – ramps, rails, safety strips on steps, lighting
  • Outdoor living spaces – ramps, rails, re-tiling
  • Bathing area – shower access, hose, taps, floor surface, rails, toilet access
  • Kitchen area – cupboard access, flooring, taps
  • Bedrooms – wardrobes, flooring, doorways
  • Internal areas – corridors and stairways
Equipment Assessment & Trial

An equipment assessment covers a range of supportive equipment.

  • wheelchairs and scooters, mobility equipment – walkers, walking sticks, crutches, rollators.
  • bathing equipment – shower commode, toilet frame, bath transfer bench, shower stool;

    toileting equipment – raised toilet seat, bedside commode;

  • daily living aids – cutlery, pick up reachers, dressing, book holders
  • chairs – recliner, static
  • falls prevention devices – lighting options, strengthen muscles, reflective strips, removal of hazards
  • lifters and hoists, beds and accessories – electric, bed rails, bed wedges, heel elevators, bed tables, monkey bar
  • pressure care – air and foam mattresses, cushions, overlays
  • Assistive Technology – voice activated in home devices,
